Help your patients end 2015 on a positive note!
CareSource has provided many MTM opportunities for pharmacists in Ohio over the past year. One opportunity for pharmacists to make significant impacts on patient lives is by providing comprehensive medication reviews (CMRs) to patients. CareSource is looking to provide this important activity to all of their eligible MyCare (Medicare and Medicaid) patients in northeast Ohio before 12/31/2015. Ohio licensed pharmacists are able to complete them and prior authorizations are NOT required to complete these by telephone.
Below is the process to use when completing a CMR assigned to another store.
- Select Patient List tab
- Select “Add a Patient”
- Enter the patient’s last name and date of birth or CareSource ID and hit search
- The next box that appears will be a HIPAA and privacy attestation, select “Add Patient”
- Now the patient will be on the pharmacy’s list and MTM services can be provided
