• Provide comprehensive and quality pharmacy services to University of Toledo Medical Center patients, employees, and students. • Provide chronic disease management under consult agreements. • Collaborate with physicians and other healthcare providers regarding medical care of patients. • Assesses the status of the patient’s health problems and determines whether the prescribed medications are optimally meeting the patient’s needs and goals of care. • Evaluates the appropriateness and effectiveness of the patient’s medications. • Recognizes untreated health problems that could be improved or resolved with appropriate medication therapy. • Follows the patient’s progress to determine the effects of the patient’s medications on his or her health. • Consults with the patient’s physicians and other health care providers in selecting the medication therapy that best meets the patient’s needs and contributes effectively to the overall therapy goals. • Applies specialized knowledge of the scientific and clinical use of medications, including medication action, dosing, adverse effects, and drug interactions, in performing patient care activities in collaboration with other members of the health care team. • Supports the health care team’s efforts to educate the patient on medications and provides other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization. • Refers the patient to his or her physician or other health professionals to address specific health, wellness, or social services concerns as they arise. • Maintains knowledge of latest pharmacy products and all governing laws. • Maintains contact with other department heads, medical staff, nursing staff and others to determine needs, resolve problems, improve processes, and promote effective drug therapy. • Maintains all patient data in a confidential manner. • Utilize effective communication to provide excellent customer service. • Actively participate in performance improvement activities as it relates to job duties. • Accurately prepare and dispense prescriptions in accordance with laws of the Ohio State Board of Pharmacy and Federal Pharmacy Regulations • Provide immunizations to patients when appropriate in accordance with Ohio State Board of Pharmacy. • Ensure accurate, compassionate, and concise medical information is provided to patients as appropriate. • Precepts, directs, and coordinates learning experiences and activities for pharmacy residents and pharmacy students. • Serves as a resource to provide health information for the University of Toledo students and staff while mentoring and teaching students. • Provides experiential and didactic instruction to professional and graduate level students. • Assists with educating and training departmental staff. • Assists pharmacy students in implementing knowledge learned in the classroom into real world experiences and developing critical thinking skills. • Provides timely feedback to all learners and completes all learning experience evaluations in a timely manner. • Serve as a positive role model for future pharmacists and devotes the necessary attention to the development of students. • Develop educational materials for patient use. • Responsible for program development, continuous quality improvement, and reporting measures. • Participates in the development and implementation of policies, procedures, and staff development. • Assists in evaluation of effectiveness of pharmacy services and oversees initiatives to improve patient satisfaction. • Participates in University outreach events to promote pharmacy services and initiatives. • Perform other duties as requested by others including Pharmacy Manager, Director of Pharmacy and/or Chief Pharmacy Officer. • All other duties as assigned. |
