The Ohio State University (OSU) College of Pharmacy invites applications for the Medication Reconciliation Pharmacist Preceptor role. This is a part-time staff pharmacist position at the College of Pharmacy under the direction of the College’s Medication Reconciliation Lead Pharmacist and Director of Experiential Education. The pharmacist will be primarily responsible for precepting PharmD students in the Medication Reconciliation program at The Ohio State University Wexner Medical Center. Through precepting, the pharmacist will provide exceptional patient care by ensuring patients’ medication histories are completed accurately and according to established OSUWMC policies and procedures. The pharmacist will also assist with rotation orientation, onboarding, and remediation of PharmD students and other College of Pharmacy duties as assigned.
